Dance Forms
Below are the 3 forms that must be filled out in order to participate in the Dance Marathon.
All paper work can be dropped off or mailed to the STRIVE Center at 28 Foden Road, South Portland, ME 04106 prior to March 3rd.
You may also bring the registration forms to the USM Sullivan Gym on March 3rd but they must be completely filled out. It is recommended to pre-register so your check-in on March 3rd is quicker.
